Depok, 29 November 2025 — The Faculty of Public Health (FPH), Universitas Indonesia (UI), once again held an Open Doctoral Promotion Session for the Doctoral Program in Public Health Sciences on 29 November 2025. The session formally conferred Duma with the degree of Doctor of Public Health Sciences after the successful defense of the dissertation entitled “Analysis of the Potential Cost Reduction for Chronic Kidney Disease Patients Covered by BPJS Kesehatan through Early Diagnosis and Prompt Treatment.”
The dissertation addresses a strategic issue related to the sustainability of financing under Indonesia’s National Health Insurance (Jaminan Kesehatan Nasional/JKN), particularly the cost burden of chronic kidney disease (CKD), which is classified as a catastrophic disease with a very high contribution to total insurance claims. In the presentation, the candidate explained that the surge in CKD-related expenditure—especially at advanced stages requiring hemodialysis—has been one of the main drivers of BPJS Kesehatan’s claim ratio exceeding 100 percent in recent years.
Using big data analysis of BPJS Kesehatan claims from 2023–2024, the study demonstrates that the implementation of early diagnostic strategies using Cystatin C testing, combined with prompt pharmacological treatment such as Sodium-Glucose Cotransporter-2 inhibitors (SGLT2i) and Renin–Angiotensin System inhibitors (RASi), has significant potential to slow the progression of CKD toward end-stage disease. The findings indicate that this preventive approach not only reduces the number of patients requiring hemodialysis, but also generates substantially greater cost efficiency compared to the management of advanced-stage CKD.
More specifically, the results show that the costs of screening and early intervention are considerably lower than the annual cost of hemodialysis per patient. With targeted screening among individuals aged 60 years and above—both with and without comorbidities such as diabetes mellitus and hypertension—the potential cost savings could reach trillions of rupiah annually. This underscores that evidence-based promotive and preventive strategies are key to safeguarding the financial sustainability of the JKN system.
The doctoral promotion session was chaired by Prof. dr. Amal Chalik Sjaaf, S.K.M., Dr.PH. as Head of the Examination Committee, with Prof. dr. Ascobat Gani, M.P.H., Dr.PH. serving as Promotor, and supported by Co-Promotors Prof. dr. Ali Ghufron Mukti, M.Sc., Ph.D., AAK. and dr. Yassir, SpPD-KGH. The board of examiners also included dr. Soewarta Kosen, M.P.H., Dr.PH.; Dr. dr. Andreasta Meliala, DPH., M.Kes., MAS.; and Dr. dr. Gea Pandhita, Sp.N., M.Kes.
Based on the successful defense of the dissertation, Duma was declared to have passed and was awarded the degree of Doctor of Public Health Sciences. Duma is the 27th graduate of the Doctoral Program in Public Health Sciences in 2025, the 366th graduate of the program overall, and the 481st doctoral graduate of FPH UI. (wrk)
